Why does a radiator cap matter?

Because it sets the pressure the whole system runs at, and pressure is what keeps the coolant liquid above its normal boiling point. A cap with a tired spring or a perished seal lets pressure escape, the coolant boils sooner, and the engine overheats on a hill even though the radiator and pump are fine. It is one of the smallest parts in the system and one of the most common causes.

How often should coolant be replaced?

By the interval in the vehicle manual — and in this climate the severe-service column is usually the right one to read. Coolant is replaced because its corrosion inhibitors are spent rather than because it has stopped freezing or boiling, so the schedule is about time and heat exposure as much as mileage.
